SEM guides and supports the implementation of multiple healthcare practice models and frameworks, including the Patient-Centered Medical Home, Integrated Behavioral Health, and Learning Health System. SEM’s approach to Healthcare Model Implementation involves three major steps: 1) a readiness assessment process to determine organizational status, strengths, gaps, and barriers to implementation using proprietary readiness assessment tools; 2) the development of a capacity-building plan to develop the infrastructure and necessary skills to prepare the organization, its leaders, and teams for implementation; and 3) a tailored adoption and adaptation process to implement the model or framework of interest.
